During the Fourth Shinobi World War, Ao worked alongside Inoichi to relay battlefield reports and communicate Shikaku’s strategic directives. Through Inoichi’s telepathic abilities, these strategies were successfully transmitted to every division of the Allied Shinobi Forces engaged in battle against the Akatsuki.

Although not directly involved in combat, Ao’s presence at the command center was crucial. His sensory abilities helped detect enemy threats, enabling Shikaku to devise effective plans to guide the Allied Shinobi Forces in defeating their foes.

Even Madara was impressed by the cooperation among the five great nations that formed the Allied Shinobi Forces to thwart the Mugen Tsukuyomi plan.

Realizing that there were key figures controlling the Allied Shinobi Forces from the command center, Madara decided to neutralize the strategic hub. Alongside Obito, Madara used the Juubi to fire a Bijudama aimed at obliterating the command center and everyone inside it.

The massive explosion destroyed the command center and claimed the lives of nearly everyone within, including Shikaku and Inoichi. However, amidst the devastation, a miracle occurred: Ao survived. Nevertheless, he did not escape unscathed—he sustained serious injuries to various parts of his body and lost his left arm due to the deadly attack.

In Boruto episode 178, Ao is shown attending the commemoration ceremony for the Fourth Shinobi World War in Konoha. He paid tribute to his fallen comrades, remembering their immense sacrifices for the peace of the Shinobi world.
